No-show Calculator: What Patient No-shows Cost Your Clinic
Free patient no-show calculator for clinics, dentists and doctors. Work out your no-show rate and how much empty appointments cost every month and year, then cut no-shows with appointment reminders and a clear no-show policy.
Your clinic numbers
Check your appointment book for the last 4 weeks. Rough numbers are fine.
How to find your no-show rate: count missed appointments last month, divide by total booked appointments, then multiply by 100. Late cancellations you could not fill also count.
Estimated cost of no-shows
lost every month from empty appointment slots
That is about 0 a year back in your clinic.
Estimate only. Formula: appointments per day x no-show rate x days open x appointment value. Real results depend on your patients, your reminders and how fast you refill empty slots.

Why patients do not show up
The 4 most common reasons for no-shows
They forgot
The appointment was booked a week ago and nobody reminded them the day before.
Plans changed
Something came up, and cancelling felt like more effort than simply not coming.
Unclear details
They were not sure about the time, the location, the fee or what to bring.
Low commitment
The booking was casual, with no confirmation step and no easy way to reschedule.
No-show cost calculator for clinics and doctors
An empty appointment slot still costs you rent, staff salaries and the doctor's time. This free no-show cost calculator shows how much revenue your clinic loses each month and each year when patients book but do not turn up, and how much you could win back.
How to calculate your no-show rate
Take the number of missed appointments in a month and divide it by the total number of booked appointments in that month. Multiply by 100 to get a percentage. For example, 60 no-shows out of 500 bookings is a 12% no-show rate. Late cancellations that you could not fill also count, because the slot was still empty.
How the cost is calculated
The calculator multiplies your booked appointments per day by your no-show rate and your days open to get no-shows per month. It multiplies that by your average appointment value to show lost revenue, and by appointment length to show doctor hours wasted. Change any number to see the effect.
How to reduce patient no-shows
- Send two reminders. One a day before and one 2 to 3 hours before the appointment, on WhatsApp or SMS. Patients reply more on WhatsApp than to calls.
- Ask for a confirmation. A simple "Reply 1 to confirm, 2 to reschedule" turns a passive booking into a commitment.
- Make rescheduling easy. If moving an appointment takes one message, patients move it instead of skipping it.
- Keep a waiting list. When someone cancels, message the waiting list right away to refill the slot.
- Share clear details. Send the location pin, fee, timing and what to bring in the booking message.
- Track no-shows every month. A clinic CRM shows who missed, so your team can follow up kindly and rebook.

Appointment reminder message templates
Appointment reminders are the simplest way to reduce no-show appointments. Copy and edit these:
1 day before: "Hi Sara, this is a reminder of your appointment with Dr. Ali tomorrow at 5:00 pm at City Skin Clinic. Reply 1 to confirm or 2 to reschedule."
2 hours before: "See you at 5:00 pm today. Here is our location: [map link]. Please arrive 10 minutes early."
After a no-show: "We missed you today. Would you like to book a new time? Reply with a day that suits you."
Sample no-show policy for a medical office
A short, clear no-show policy sets expectations without upsetting patients. Example:
"Please give us at least 24 hours notice if you need to cancel or reschedule, so we can offer your time to another patient. After two missed appointments without notice, we may ask for a booking deposit for future visits."
Share the policy when the patient books, show it at reception and include it in the reminder message. Follow your local rules before charging any fee.
Fill your empty slots
We set up automatic WhatsApp reminders, confirmations and waiting-list messages so fewer patients miss their appointment.
FAQ
No-show calculator: common questions
What is a normal no-show rate for a clinic?
It changes a lot by speciality, city and how far ahead patients book. The best number to use is your own: count last month's missed appointments and divide by total bookings.
How do I calculate the cost of no-shows?
Multiply your no-shows per month by the average value of one appointment. This calculator does it for you and also shows yearly loss and doctor hours wasted.
Do appointment reminders really reduce no-shows?
Reminders help many patients who simply forgot, and a confirmation step helps you spot cancellations early. How much they help depends on your patients, so measure your no-show rate before and after you start.
Should my clinic charge a no-show fee?
Some clinics take a small booking deposit for long or high-value treatments. Explain the policy clearly when the patient books, and follow your local rules.
Is this calculator free?
Yes. It is free with no sign-up. If you want a personalised plan, add your WhatsApp number and we will send it to you.
What is a good no-show policy for a medical office?
Ask for 24 hours notice to cancel, explain it at booking, and only consider a deposit after repeated no-shows. Keep the wording polite and follow local rules.
When should appointment reminders be sent?
Most clinics send one reminder a day before with a confirm or reschedule option, and a second short reminder 2 to 3 hours before the visit.
